The following forms are available on this website. There are two methods for submitting these forms to Council:
The Annual Declaration Form is mailed to landowners in March. Forms are also available by request from the Council office.

- Required by the Private Managed Forest Land Act
- Submitted by the landowner as application for Managed Forest land class
- Includes landowner commitment to adhere to the Act and Regulations

Management Commitment Amendment (Updated July 2006)
|
- Add land to existing Management Commitment
- Remove a portion of land from existing Management Commitment
- Change specifications of existing Management Commitment
|
- For existing managed forest landowner who has Management Commitment on file with the Council
- To amend existing Management Commitment
- Usually applies to addition of Managed Forest land area

Withdrawal of Management Commitment |
- Withdraw Management Commitment and exit from Managed Forest Program
|
- Identifies lands to be withdrawn from an existing Management Commitment on file with the Council
|
|
Annual Declaration (Updated Feb. 2010)
|
- All Managed Forest landowners submit to Council by May 1 of each year
|
- Outlines forest activities including harvesting, destroyed timber, road construction, road deactivation and reforestation on the Managed Forest land for the previous year
|
|
Notice of Sale
|
- Notify Council within 30 days of the sale of private Managed Forest land
|
- Landowner notifies Council when a property is sold, and provides contact information for new owner
|
|
Inquiry/Complaint
|
|
- Can be submitted by member of the public or Managed Forest landowner
- Supplies information for Council to determine nature of inquiry or complaint, as well as contact information
|
Environmental Incident Self Report |
- To report a slide or other incident that may affect a stream or other key public value
|
- Landowner notifies council when an environmental incident occurs on managed forest land
|
Authorization to Act as Agent |
- To appoint an agent to act on behalf of the property owner(s)
|
- Landowner can appoint an agent or one owner of a multiple owner property to act as agent with respect to the management commitment. Form is signed by all owners.
